If your vision insurance carrier is not listed above, please feel free to contact us and we will gladly check to see if we can bill your insurance. We ask that you provide us all the necessary insurance information at the time the appointment is scheduled. This allows us ample time to verify all exam and hardware benefits and identify any problems before your appointment. Please inform us of any changes to your insurance that may have occurred since your last visit.
